This Team Registration Form and the included Liability Waiver MUST be fully completed and submitted to THE CITY LEAGUE before the 1st week of play.
Completed Registration Forms with payment can be mailed to 1108A South Boyle Ave, St. Louis, MO 63110. Team Captain will be contacted for confirmation once the league has received your teams registration.
