Urbandale Softball - Hall of Fame Application
Use this form to nominate any current or former player to the Urbandale Sports Association Softball Hall of Fame. All nominee's will be reviewed by the Urbandale Sports Association Board of Directors and voted upon for approval. All approved nominees will be added to the Hall of Fame list as well as have their name engraved on a plaque located at Walker Johnston Softball Complex. Additionally, new members will receive a letter of congratulations and a certificate of membership to the Hall of Fame.
