This document contains names and contacts of all club coordinators in the WMYLC league.
WMYLC Code of Ethics
I ________________________________________ as a player, coach, parent, volunteer, officer, and/or
official do hereby affirm and declare that I will uphold the principles of good sportsmanship and fair play
endorsed by U.S. Lacrosse (addressed in the U.S. Lacrosse Parents Guide to the Sport of Lacrosse,
National Federation Rule Books, and NCAA Rule Books).
WMYLC Waiver Form
The WMYLC waiver form is required for all participants in WMYLC activities. One form must be filled out for each general activity group (example: one waiver filled out for spring outdoor season, and another for summer league). Each participant (players and coaches) must fill out and sign the forms. Participants under 18 years of age must have the forms completed and signed by a parent or guardian. Forward completed form to the club coordinator for your team.
