This form must be read and returned to your Coach once you have been selected for a team. You will not be allowed to play until this form is turned in!
Medical Card
Please turn this in to your coach once the team has been selected.

These forms discuss all of the requirements to be eligible to participate this season. Please read them carefully, sign the back, and return them to your coach.
