  • Coaches Clearances
    Coaches, Here are the clearance forms that you will need to coach in the league. The criminal you can do on line. After you get your criminal back, send in a copy with your child clearance request. You can register for your FBI clearance with the attached form or online at cogentid.com then get your prints done at Goin Postal in Connellsville.

  • Coaches Annual Clearance Statement (Act 24)
    If you already have a criminal and Child Abuse Clearance on file, you need to complete this form to affirm that you have had no issues since your criminal orchild abuse form was issued.
