CHECKLIST
This checklist has everything listed that we will need by football equipment handouts. If you have multiple children, one emergency medical form will be sufficient, as well as one Code of Conduct form.

Football Offline Registration Form
Complete this form if you have not already registered for football online.

Emergency Medical Form
This form is required by all athletes in our program.

Physical Form
This is a standard physical form. You can use this for your son or daughter's physical or a note from the Doctor stating your son/daughter is clear to participate in their activity. If football, it should state either Flag level football or Tackle football. If cheer, it should state that he or she is clear to participate in cheerleading.

JSFC Eligibility Form (Tackle Football Only)
This is for all athletes participating in tackle level football. This form will confirm eligibility. This must be signed by player, parent or guardian and league rep.

Code of Conduct - Athletes and Coaches
This is a required form. We must have a current Code of Conduct on file for all families in our program. Please review with your family this information.

Program Shout Out Ad Form
If you've seen our season program, you know it is one of kind. Be part of this program for your student athlete with sponsored ad wishing them luck, love and anything else! Parent, grandparents, guardians and family members. We'll even take ads from your pets. Ad placement closes 08/25 - so be sure to get your form and payment in by the deadline.

Concussion Awareness
Informational document on Concussion awareness. Impact testing will be scheduled for all players who have not been tested in the last 12 months.
