2016 RINK Physical form
All children playing RINK (Ages 6 -8) football are required to have a sports physical during the 2016 playing year. The physical must be completed within the 2016 calendar year.

2016 PONY MIDGET Physical Form
All children playing PONY or MIDGET (Ages 9 - 12) football are required to have a sports physical during the 2016 playing year. The physical must be completed within the 2016 calendar year.

2016 YCYFA Coach Code of Conduct
Each Coach choosing to coach for the WYBC and YCYFA is required to have a Code of Conduct on file.

2016 YCYFA Player/Parent Code of Conduct
Each player taking the field will need a Code of Conduct signed and on file with the YCYFA Board and the WYBC Football program. Please have your child sign the form along with a parent.
