    Must be filled out by each baseball player. Uniforms will not be distributed to players who have not filled out this form.

  • YYR Coach Code of Conduct
    All baseball coaches must sign and submit this Code of Conduct.

  • Registration Payment Voucher
    Please send this voucher along with your registration payment to King & Associates (address is on form). Please make sure to write your player’s name and team (i.e. Derek Jeter/12U) on the check.

  • YYR Scholarship Application Form
    Attached you will find the York Revolution scholarship application. In addition to the application you will need to provide the following documents, some of which are noted on the application as well:

    ·Prior year’s W-2

    ·Prior year’s Federal Income Tax Return

    ·Valid driver’s license

    ·Most recent pay stub

    ·Current employer’s name and contact information, your job – responsibilities and income

    ·Professional and personal references with their contact information – one of them must be your currently employer, if employed

    ·A written summary detailing household income vs. expenses on a monthly and annualized basis along with a narrative explaining in detail why your player should receive the scholarship funding

  • Small Games of Chance Overivew
    An overview from the Treasurer's office of what is legal under our Small Games of Chance license. Please send an email request for a copy of our license. We must receive a letter from any venue where you plan to operate small games of chance, naming them as the host venue. That letter should be received by YYR three weeks prior to your event.
