PLAYER REGISTRATION
Required forms for player registration include:
1. A photo copy of the player's birth certificate. 2. Player registration form. 3. Parent/Player/Fan Code of Conduct form signed by parent/guardian and player. 4. Parental Waiver Form 5. Player Registration, Parental Waiver form and Code of Conduct forms are located in the "Handouts" section in the Main Menu on the left.
1. Volunteer coaches application is located in the "Coach's Box" in the Main Menu on the left. 2. Coaches Code of Conduct form is located in the Coach's Box" in the Main Menu on the left. 3. A photo copy of your current, valid picture driver's license or another current, valid gov't issued picture ID. 4. A photo copy of your Rutgers S.A.F.E.T.Y. Clinic Certification card. 5. All coaches will be subjected to a background check.
VOLUNTEER REQUIREMENTS
Required form(s) for those who wish to volunteer include:
1. Volunteer application is located in the "Handouts" section located in the Main Menu on the left. 2. Volunteer must provide a copy of their current, valid, picture driver's license or another current, valid, picture gov't issued ID. 3. All volunteers will be subjected to a background check.
