We have scheduled the Apopka Raptors to get their physical completed by St. Germain Chiropractic Clinic in Apopka. The cost of a physical is $10 per participant.
1. All physicals need to be dated after January 1, 2012 in order to be valid and they must be on the physical form attached. If you have the information transferred by your physician please make sure the doctor has signed/stamped the form.
2. The physical form will be available at the front desk and a copy is available to download if you need it for your private physician.
3. Make sure that you keep the original physical form. Turn in a copy to your coach or team mom. Let the receptionist know that you are from the Apopka Raptors. Do not assume they know this information as other sports teams come to their office to get physicals.
4. Please note that players/cheerleaders will NOT be able to participate in ANY practice at the beginning of the season without the physical. NO EXCEPTIONS!!!
