CHANGE IN PHYSICAL FORMS AND WHEN THEY ARE DUE

There has been a change to the physicals and when they are due this year.  All physicals are due 6 weeks prior to the start of the season.  The physical form is being passed out at sign-ups or you can print it from our handouts/forms page.  All Physicals must be completed on the DAMF form.  No physical form can be signed before June 1,  2018.

NO ATHLETE WILL BE ABLE TO PRACTICE WITHOUT THEIR PHYSICAL FORM COMPLETED.  IF PHYSICALS ARE NOT COMPLETED BY WEIGH-INS YOU WILL NOT BE ABLE TO PLAY THIS YEAR!!!

If your insurance does not cover an additional physical yet, you can go to Urgi-Care in Blairsville or any Med Express and they offer physicals for around $30-$35.
