FOREST PARK BLACK HAWKS YOUTH FOOTBALL & CHEER

Maryland State IDs are required for all football players!  They can only be obtained from the Maryland Vehicle Administration (MVA).

KNOW BEFORE YOU GO!

Here are the requirements for obtaining a NEW ID:

1. Proof of Age
   1. Original Birth Certificate  (No Copies!) OR
   2. Unexpired U.S. Passport (No Copies!)
2. Original Social Security Number (SSN) Card (No Copies!)
3. Parent OR Legal Guardian must accompany the child (guardian must have legal documentation of guardianship)
4. Proofs of Residence (at least 2 of any below)
   1. Motor Vehicle Registration
   2. Mortgage/Lease
   3. Federal or State Taxes
   4. Life Insurance Policy
   5. Utility Bill (BGE, telephone, cable,)
   6. Checking or Savings Account Info
   7. Major Credit Card Bill
   8. Voter Registration Card
