PLAYER REGISTRATION SYSTEM
HOW TO REGISTER A PLAYER
1. NEW PLAYER REGISTRATION FORMS
Registration forms are available from either Elaine Pritchard, the League/Registration Secretary, Football Reception at Debdale and Kirkby Bentinck or the Notts FA.
Players who were not registered with your Club during season 2007/08 must complete a new player Registration Form.
If the player was registered with another Club by the Notts FA, you should not need a photograph or birth certficate - if in doubt, please check with the Notts FA. However, we strongly recommend that your Club verifies the date of birth of all new players to your Club by checking an original birth certificate and any other Government documentation attesting to the players date of birth.
If the player has not previously been registered by the Notts FA, you will require a photograph and proof of date of birth (birth certificate etc).
Photographs must be passport size and glued to the Registration Form. The player must be able to be identified from the photograph. No hats or sun glasses.
PHOTOCOPIED FORMS WILL NOT BE ACCEPTED.
RE-REGISTRATION FORMS
Re-Registration Forms have now been issued to Club Secretaries.
You may re-register any of the players listed on this form by completing the pro-forma and obtaining the signature of the player and, if the player is of school age, the signature of the Parent/Guardian.
Any players not on this list who wish to register with your Club must complete a New Registration Form - please do not add players to Re-Registration Forms, they will be dis-regarded.
You may submit only one re-registration form (ie - register as many players as possble on this form - additional photocopied forms will not be accepted).
Re-Registration Forms will not be accepted after 30th September 2008.
2. COMPLETING FORMS
Please ensure that Re-Registration Forms and New Player Registration Forms are completed clearly in BLOCK CAPITALS in blue or black ink - incomplete, inaccurate or ineligible forms will be returned and therefore delay the processing of the registration.
3. NUMBER OF REGISTERED PLAYERS
Mini Soccer & Under 11's 9 v 9
Each team must not have a squad greater than double the size of the team ie for 7 v 7 you may have a maximum of 14 registered players and for 9 v 9, no more than 18.
You may de-register players who are no longer actively involved, by
completing a De-Registration Form (available from the League/Registration Secretary or Football Reception) and returning the existing player registration card.
Players wishing to play for another team must be transferred using
the transfer procedure. Transfer forms also available from the League/Registration Secretary or Football Reception.
4. SUBMITTING FORMS
From 1st June to 30th September 2008, completed forms must be sent to the League/Registration Secretary at 3 Hathersage Way, Sutton in Ashfield, Nottinghamshire. NG17 2BE who will check to see that the forms have been correctly completed - incomplete, inaccurate or illegible forms will be returned and therefore will delay the processing of the registration.
From 1st October 2008 Registration Forms must be submitted directly to the Notts FA, 7 Clarendon Street, Nottingham NG1 5HS. If sending by post, please ensure that you pay sufficient postage, taking into account the new postal charges for A4 size envelopes.
During the season Registration Forms must be received at Notts FA by 12.00 noon on Friday in order to be processed to enable players to play on the Sunday immediately following.
The last date for registering a player is 16th March 2009.
5. REGISTRATION FEES
The cost of registering is 2.00 per player.
The League will invoice you for £1.00 per player.
The Notts FA will invoice you for £1.00 per player.
TRANSFERS
All transfers mut be carried out in accordance with FA Rule C2.
Transfer forms are available from the Notts FA, the League/Registration Secretary or Football Reception.
Please complete all parts of the Transfer Form clearly in BLOCK CAPITALS -incomplete, inaccurate or illegible forms will be returned and therefore delay the processing of the transfer.
Transfer Forms must be received at Notts FA by 3.00 pm on Thursday in order to be processed to enable players to play on the Sunday immediately following. The form must be accompanied by the £5.00 fee (cheque payable to NOTTS FA LTD) and the player's registration card for the team currenly holding his registration.
