KEY DATES with FINES
$ 50 LATE FEE FOR EACH MONTH LATE PER ITEM.
Fines increase by $50 each month late for each item late.
If form or forms are submitted late attach check in the amount of fine.
Each year all organizations franchised by this Conference are required to submit to the Conference Office the documents listed below.
Failure to submit to the Conference Office by the due date will result in a $50.00 fine for each pass due item. An additional $50.00 will be assessed per item for each month pass due.
All forms must be submitted to the CONFERENCE OFFICE by the due date listed. Forms can be submitted by US Mail or Hand to Conference Treasurer at the conference meeting on the DUE date. NO FAX copies accepted.

1. Complete list of your 2009 City Board and Officers. Must include: complete Name, Address, Home Phone, Cell Phone, Work Phone, FAX Phone number and e-mail address. Conference sends out minutes and out information by e-mail. E-mail is faster and cheaper. E-mail cost nothing to send or receive.

2. Two Copies of organizations Constitution and Bylaws as amended and dated.

3. Copy of Federal Disclosure Letter.

4. Copy of Statement by Domestic Non-Profit Corporation.

5. An audited financial report of previous year operation. Report must be submitted on Conference Financial form or other format as approved by Conference Treasurer.

7. Copy of State Registry of Charitable Trusts form (RRF-1).

8. Copy of Federal Tax Form 990 as filed for previous year.
