RECREATIONAL REGISTRATION REQUIREMENTS: ** Coaches: If you need players, contact your age group commissioner. Parents/players: If you are looking for a team, please sign-up online by submitting a Sign up for Soccer Form.
THE FOLLOWING PAPERWORK WILL BE REQUIRED IN ORDER TO REGISTER YOUR RECREATIONAL TEAM:
1. Completed US Club Soccer Recreational Membership Form for each player (completely filled out and signed by parent).
2. Birth Certificate for each player (only a copy of the certified state-issued birth certificate will be accepted. Hospital issued birth certificates will not be accepted per Dallas East Rules). Passports and drivers licenses are acceptable for proof of age.
3. Recreational Team Roster (completely filled out in alphabetical order), signed and dated by coach
4. Approved US Club Soccer Background Check(required for every coach/asst. coach/manager/trainer) that must be done online through US Club Soccer.
