Remember, Lightning can be a deadly situation. Before or during any game
if a LIGHTNING STRIKE is spotted or heard we immediately leave the playing field for cover. We must wait 30 minutes for the storm cell to pass. If no more LIGHTNING STRIKES are heard or seen, play can resume. If in that 30 minute time, other STRIKES occur the game will be called or suspended at that time.

Player and Parent Requirement:
JCPS Middle School Physical Form.
JCPS Parent Permission and Physical Addendum
JCPS Health and Safety Video and signature form available from school Athletic Director.
JCPS Volunteer Auto Affidavit. (If a parent or coach is providing transportation to players other than THEIR own child')

Head Coach Requirements:
JCPS Volunteer Records Check.
JCPS Health and Safety video and signature form available from school athletic director.
JCPS Volunteer Auto Affidavit (same as above)

Volunteer (Coaches and Asst. Coaches) Requiremtns.
JCPS Volunteer Records Check is required for anyone interacting with players.

Schools participating from outside of Jefferson County Schools or TEAMS with Religious Affiliation to a school must conform to that school's or
county's requirements.

ALL SCHOOLS, OTHER THAN JEFFERSON COUNTY PUBLIC SCHOOLS, MUST PROVIDE AND SHOW PROOF OF INSURANCE COVERAGE.

The Middle School Fast Pitch Softball League shall be free from any liability or cost pertaining to any accident or action resulting from participation in this league.
