2016-2017 Player Profile Form
This form contains information about a player. There are 2 options this year. You can either print and complete this form or you can do an ON-LINE REGISTATION instead by clicking here and completing an on-line form.
2016-2017 PVC Tryout Form
This form is your check list of all the items that are required for registration. It also includes select player's information that will be referenced by PVC Staff.
Gym Waiver
This form is required by the GYM. All players (open gym, training, developmental, camp or full season players) must have an executed Gym Waiver on file before stepping into PVC gym.
2016-2017 PVC Application Agreement Form
This form pertains to membership in the Powers Volleyball Club and must be completed by all team and developmental players.
2016-2017 PVC Participant Release of Liability Form
This form pertains to membership in the Powers Volleyball Club and must be completed by all team and developmental players.
2016-2017 USAV Medical Release Form
This form must be completed and notarized in order to play in any USAV tournaments.
Please, register for USAV membership at www.floridavolleyball.org. Registration will be opened after 08/15/2015. Club affiliation will remain UNDECIDED, until November 1st. At that point, you will need to login to your membership and update with the club of your choice. USAV Membership in Florida Region is from 09/01/2015 till 08/31/2016. Register for 2016-2017 AAU Membership
This Membership has to be active in order to play in any AAU tournaments. Registration will be opened after 08/15/2016. Please, register at www.AAUSPORTS.ORG. Please, choose Powers Volleyball Club as your club. PVC Club Code WWDBBA. AAU Membership is from 08/17/2016 till 08/31/2017.
Tryout Fee of $50.00.
Tryout fee can be paid by check payable to Powers Volleyball Club or cash. Tryout fee is non-refundable.
Deposit of $150.00.
Deposit is refundable if a player is not placed on a team or until the time a player accepts an offered position on a team. Deposit also can be paid by check payable to Powers Volleyball Club or cash.

Season Payments
Payments are due the first week of the month throughout the club season. Payments can be made by check or by credit card using Pay Pal option. If you decide to pay by check, please, make checks payable to Powers Volleyball Club. Mail your check to: 117 Oak View Circle, Ponte Vedra Beach, FL 32082. If you decide to pay via Pay Pal, please, add 3% to your payment. There will be a $25.00 late fee for all payments that are past the 7th or each month. Pay-in-full option is available as well for those who would like to take advantage of 5% off the club season tuition.
