Required Registration Player Form
Complete and sign the CYSA Registration Form. The CYSA membership form is available online as a fill-in form. If you fill out via online please print 2 copies to mail or bring to a registration walk-in sessions (Walk-in dates will be schduled soon)
Filling out the CYSA Membership Form via online:
Click on form link, once form opens type info on form. Complete the player information, primary guardian, and secondary guardian sections. Once complete, print (2copies). Most important, the parent/guardian
MUST sign the Medical and Liability Release section of the form at the bottom right of the form. This form must be signed with a "Wet Ink Signature" must be in color ink (Blue, Red), not black.
Filling out downloaded CYSA Membership Form (offline):
Click on form link, print form via your computer, fill out form with blue ink pen. primary guardian, and secondary guardian sections. Most important, the parent/guardian
