In addition to a filled out registration form that you can download above, you will need to bring the following items with you to complete registration:


1. Registration Check for $225.00 (for first child, plus $115.00 for each additional child)


2. Concession Check for $100.00 (post dated July 1st and refundable with completion of time commitment)


3. Copy of Birth Certificate (for all new players and cheerleaders)
