Softball All Ages - $120 Softball Registration Form

Additional child(ren) discount of $5 after the first child.
Fee includes: Uniform, Equipment(except baseball/softball glove)Access to Training Facility, Season End of Season party for all players.
PLEASE NOTE: Registrations received after February 8, 2013 are now subject to a $25 late fee. There are no multiple child discounts for late registrants. Late registrations must be postmarked no later than April 1st, 2013.
Any check that is returned will incur an additional $20 fee. Also the registration fee must be payed in cash or a money order.
* If this is your child's first season we need a copy of your child's birth certificate or other suitable birthdate proof. If you can not come in person then please mail it along with your signed registration.
