IS ALL YOUR PAPERWORK IN?
Please remember your child cannot start practice without the following paperwork on file:
- A copy of Original Birth Certificate
(if they participated last year, we have it)
- A recent wallet size photo
- An official Pop Warner Medical Form
(available at the Rec Center)
filled out by your child's doctor stating they are in good health
- TWO copies of your child's final report card from this school year
PLEASE TAKE CARE OF THIS RIGHT AWAY, ESPECIALLY THE DOCTOR'S FORM.
YOUR CHILD CANNOT START WITHOUT THE PROPER PAPERWORK.
THIS RULE WILL BE STRICTLY ENFORCED!!
YOU CAN DROP YOUR PAPERWORK OFF AT THE REC CENTER
PLEASE PUT IT IN AN ENVELOPE WITH YOUR CHILDS NAME ON IT
