REGISTRATION 2009
Registration dates have been set for the 2009 season. We have three dates set with a sliding fee scale. Time of registration will be 10 AM to 5 PM each day.
All registering of players will be done at the Nike Park Senior Center prior to practice beginning on August 3rd. Also all forms and fees must be turned in at the SAME time. Once you have everything come see us and your child will be placed on a team then and there.
The dates are:
June 20 Fee: $150
June 27 Fee: $150
July 18 Fee: $200
Once again we will be doing the Best Buy Raffle tickets which could refund you $50.(See fees section below)
Forms are available under the HANDOUT tab on menu. Also we will have packets available for pickup prior to registration. Look for pickup locations here.
We have arranged to have medical staff present at the registration times for sports physicals. A medical doctor will be available for sports physicals at each registration from 10-130. The fee for physicals will be $20, CASH ONLY. We will keep you updated.

Items Needed
These are the items you will need to bring to registration to completely register your child. All forms & contracts can be found at a later date under the HANDOUT menu tab.
Completed Registration Form
Completed Smithfield Contract
Completed Medical form (Physical must be in the current 2009 year)
3 copies of childs end of 2008/2009 school year report card showing final grades
Copy of original birth certificate (If you played in 2007 or 2008 you will not need to provide this as you are already in the computer system)
Registration Fee paid in full
Child must be present to be weighed.

Fees
In an effort to make it easier again on parents and volunteers, we are attempting to minimize fundraising events this season. We will keep the registration fee to $150 for those registering in June. In July the fee will rise to $200. In August even higher to $250. Of this fee $50 is returnable to you through the Best Buy Raffle ticket event.
Returnable how? When you register and pay the registration fee you will be given 10 raffle tickets worth $5/each. You may sell these tickets to others and keep the $50 you collect or keep the tickets for yourself. All ticket stubs need to be returned for chance at prizes.
This was a HUGE success last season and we are currently we are looking at doing an exact repeat with a grand prize of $1000 gift certificate to Best Buy. The prize list will be finalized by the first registration. You may also purchase additional tickets at anytime to resell or keep for yourself.
So your child can actually play SPWF for $100 this year which is a real bargain in today's economy.
We the impending fees being forced upon us by the school system we may deem it necessary to have another mandatory fundraiser.
An optional fundraiser we do is the PUNT, PASS & KICK event combined with Family Fun Day and Team Pictures. This is a fun event to raise money for the association and win prizes. We hope you will participate this year. The smallest amount of pocket change helps the kids stay on the field and off the streets.
