WHO CAN PARTICIPATE? 5-12 YEAR OLDS THAT WANT TO LEARN CHEERLEADING & MEET NEW FRIENDS WHILE LEARNING HOW TO FOLLOW DIRECTIONS FROM A COACH & FOLLOW RULES OF THE GAME.
(AGE IS BASED ON THE AGE OF THE CHILD AS OF AUGUST 1st)

REQUIRED ITEMS FROM EACH CHEERLEADER:
* ORIGINAL BIRTH CERTIFICATE - TO BE RETURNED AT THE END OF THE SEASON
(NOTARIZED COPIES CAN BE PURCHASED FOR $10 FROM THE COURTHOUSE)
* COPY OF FINAL REPORT CARD FROM THE LAST SCHOOL YEAR
* POP WARNER PHYSICAL FORM (download from "rules & regulations" section of this website)- COMPLETED BY A PARENT & DOCTOR
*SMALL PHOTO OF CHILD (APPROX. 1" X 1" - FACIAL PHOTO)

CHEER COACHES -- PLEASE READ !!
Thank you for volunteering your time on & off of the field.

We have two forms that you need to review: You can find these forms on the "Rules & Regulations" section of this website.

**Coaches Code of Ethics: This form is a guideline for what Chapel Grove Optimist Club expects from each coach. Please review this sheet & sign - please turn into the Cheerleading Rep.

**Coaching Application/Pop Warner Form -- this form is required to be completed along with a copy of your drivers license. Chapel Grove Optimist Club is required to do a background check on you before final approval for coaching.

We hope you understand that we must be thorough when it comes to working with children. Thank you in advance.
