Organization Information for Parents

Registration Dates:  Saturday, May 13; Saturday, June 3, Sunday, June 11; Saturday, July 8  -  9:00-12:00 @ the Mikey Sedlak Building (Dolan Street)

Last Day to register your child/children for the program is July 8^TH.   Applications will still be accepted during the first two weeks of practice – late fees will apply.

All monies and paperwork are due July 8th, including post-dated checks

Parents are to contribute a minimum of 10 hours for work bond prior to 10/15/2017

Uniform Equipment fee $250 postdated check 11/15/2017

Mandatory Raffle fee $150 due 7/8/2017

Birth Certificate Original and copy

2” x 2” picture of child, school picture preferred, NO sport picture allowed

Player contract

Final report card of child - 2 copies

Physical form completed and signed by doctor

Football

Uniform Jerseys - $75.00 each, must be order by July 8th to be personalized

Any uniform ordered after July 8th is NOT guaranteed personalization nor delivery by first game

Any uniform ordered after July 8th will have an expedited delivery fee of $25.

Cheer Non-Competitive Levels 5 to 8 years old

Game Uniform package is $75.00, must be paid by July 8th

Competition bow fee $15.00 due August 15th

Competition t-shirt fee $15.00 due October 1st
