If you choose to register your child for both Football and Basketball at the same time you are entitled to a $10 reduction in the total cost of registering your child in both sports programs.  You are entitled to this reduction for each individual child who is registered in the J-Hawk Football AND Basketball Leagues.
 

This offer DOES NOT APPLY if you are registering 1 child in one sports program (e.g. older child in football only) and a second or more child(ren) in another sports program (e.g. younger child(ren) in basketball only).

 

If you elect this option, please fill out the Football League Registration form AND the Basketball League Registration Form (both of which are in linked files below) AND this form (which you can download by clicking on the linked file above) and MAIL ALL 3 FORMS with your check to our league P.O. Box listed on the basketball and football registration forms.  The same 3 forms must be submitted for each different child you elect to register in both sports programs under this offer.

 

Electing this option does not waive the administrative late fee of $20 required for all football registration forms postmarked after August 10th.
