In order for the President to nominate and install effective team managers for the 2010 season, we would like to have some feedback from your experiences with the manager during the past season. Please fill in the information as honestly as possible. The manager will not see the forms, however, as anonymous comments are difficult to justify, we would like your name and e-mail so that the league may follow up, if necessary. This is one of the only ways we have of obtaining information to use in selecting next year's staff. If you have more than one child in the program, please complete a separate form for each manager.  Your response will be reviewed by the Little League President and will be held in strict confidence.  Remember, your feedback will make a difference and improve our program!
