COACH'S CERTIFICATION OVERVIEW
This link describes what is required by all new coach's to complete the coaching certification process.

CRIMINAL BACKGROUND CHECK FORM (Step 1)
STEP 1: This for must be sent in prior to coaching. Download the form, fill it out and hand in to the HEAD COACH of the team. No Fee required.

CHILD ABUSE CLEARANCE FORM (Step 2)
STEP 2: This form can be filled out at the same time as Step 1. Download the form, fill out, and hand to the HEAD COACH. No Fee required.

SAFE ENVIRONMENT TRAINING SCHEDULE (Step 3)
Safe Environment Training Schedule for Fall/Winter 2009-10. Each coach is required to complete this training prior to coaching. Click on the icon for the Training class schedule. After following this link to the VIRTUS web site:
1) Select REGISTRATION
2) Select VIEW LIST OF SESSIONS
3) Select PHILADELPHIA from the drop down list
You then can see details about each session that is offered.
There is no cost for this seminar but PRE-REGISTRATION IS REQUIRED.

COACH'S CERTIFICATION CLASSES (Step 4)

Each coach is required to complete the Archdiocese Coach's orientation class within a year after they start coaching for OLG. Click on this icon for the schedule and locations. The cost is $15.00 and there is NO pre-registration needed.

COACH'S AFFIDAVIT
This form must be filled out and signed by each coach Head and Assistant and then handed in with the Team roster to Region 20.
INSTRUCTIONS:
1) Fill out form
2) Hit SUBMIT button
3) A copy will be sent to your email address.
4) Print off copy, sign and give to Head Coach or Athletic Director

OLG BLANK ROSTER
Blank CYO Roster to be completed by the Head Coach. Form must be filled out completely. Signed by the coach and passed on to the Athletic Director.
