JCMS Baseball League requirements are highlighted in RED and are to be turned in to league administrators at or before our preseason coaches meeting.

JCPS requirements are in BLACK and are the responsibility of the Athletic Director/coach at your school and should be turned in to them.

All FORMS can be DOWNLOADED from the HANDOUTS section of this site, with the exception of the Health and Safety video and signature form which can be accessed through the LINKS section.

Player/Parent Requirements:
FORM(D) JCPS Middle School Physical Form.
FORM(E) JCPS Parent permission and physical addendum.
JCPS Health and Safety video and signature form available in "Links" section of the menu.
FORM(F) JCPS Volunteer Auto Affidavit.(If providing transportation to players other than your own child)

Head Coach Requirements:
FORM(I) JTown Ball's Code of Conduct.
Signed Roster prior to first game.(See rules common to all divisions for specifics)

FORM(J) JCPS Volunteer Records Check.(JCPS employees provide proof of employment) 
JCPS Health and Safety video and signature form available in "Links" section of the menu.
FORM(F) JCPS Volunteer Auto Affidavit in Handouts.(If providing transportation to players)
FORM(K) KHSAA Pitching record/rules form.(must be kept up to date and available for review at every game)

Volunteer(ie Asst. Coaches) Requirements:
FORM(J) JCPS Volunteer Records Check is required for anyone interacting with players. (JCPS employees just need to provide proof of employment, ie JCPS email address)
