Coach's Clearance Information Handbook

This document will provide you with all the information you need to secure the required clearances to be an OLG CYO Coach.

PA CRIMINAL BACKGROUND CHECK

Please bring your completed form to the OLG Office.

CHILD ABUSE CLEARANCE FORM

Go to the website and complete the form.   The results of this clearance will be mailed to you.  After receiving the results, please submit a copy to the OLG office.

VOLUNTEER AFFIDAVIT

Please complete the OLG Volunteer Affidavit if you have been a resident of PA for the last 10 consecutive years.  If you have not been a PA resident for the past 10 years, you must complete a Federal Fingerprinting Background Check.

Each coach is required to complete Safe Environment Training prior to coaching. Click on the icon for the Training class schedule. After following this link to the VIRTUS web site:
1) Select REGISTRATION
2) Select VIEW LIST OF SESSIONS
3) Select PHILADELPHIA from the drop down list

You then can see details about each session that is offered.

There is no cost for this seminar but PRE-REGISTRATION IS REQUIRED.

Bishop McFadden Letter
Each coach must complete this 1 hour training session. Check the schedule and follow the instructions to schedule the class.   Click the logo for registration information
Fall coaches must complete this by September 20th
Winter Coaches must complete by December 1st
Spring Coaches must complete by April 1st

Each coach is required to complete the Archdiocese Coach's orientation class within a year after they start coaching for OLG. More information about orientation classes to follow.

COACH'S HANDBOOK
This document contains information that all OLG coach's will need before and during the season.

This form must be filled out and signed by each coach Head and Assistant and then handed in with the Team roster to Region 20.
INSTRUCTIONS:
1) Fill out form
2) Hit SUBMIT button
3) A copy will be sent to your email address.
4) Print off copy, sign and give to Head Coach or Athletic Director

OLG BLANK ROSTER
Blank CYO Roster to be completed by the Head Coach. Form must be filled out completely. Signed by the coach and passed on to the Athletic Director.
