Information from the Parent Meeting

1. Required Forms:Fulton County physical form is required to be completed BEFORE your child can practice. Please note, it must be the Fulton County form from this website. Please have the physical form completed before October 2nd.
   1. Fulton County Physical Form
   2. Utility Bill
   3. Birth Certificate
   4. Copy of Progress report showing the name, grade and school your child attends ( can be obtained from Home Access)
2. Practices schedules will be posted soon. Expected two 1.5 hour practices per week
3. Conference games will begin the first week of November
4. Program Fees are $450 per child due in full by October 16th. You will receive an invoice the week of October 9th. If you do not receive an invoice, please contact us at info@ahsjrladyraiders.com. You may pay by check or credit card or check.
5. The program is a 501(c)3 non-profit organization. If you or others would like to make a donation, we can provide a letter for you. 
6. A shooting shirt is included in your fee along with a uniform for your use. The shooting shirt will be yours to keep.
7. We will consider a bulk shoe order based on interest.

• Student's first semester progress report (Can be downloaded from HomeAccess)-MUST SHOW Student name and address
