All-Star Manager: You or a team representative assigned by you must complete this form to receive reimbursement up to the amount of donations your team collects for this season’s All-Star tournaments.
Complete column 1 with the payee’s name. Provide receipts for hotel and gas expenses dated for the day before to the day after any tournaments in which your team participated. Receipts must be sorted according to payee using envelopes or other means. If your team purchases uniforms from Camden Babe Ruth, you need to maintain a list of the amount paid by each family if you would like the league to issue individual checks to your families. If you purchase uniforms from another source, you must submit receipts in order to be reimbursed.
Donations by check payable to the league must be disbursed to teams using the voucher form and submitted receipts.
Donations made by cash or check payable to any entity other than the league (i.e. teams or individuals) need not be handled through the league. Please note that these types of donations do not enable the donor to claim any tax benefits by using the leagues tax ID number.
