HOW TO REGISTER A TEAM FOR SEASON 2009/10
FOR NEW CLUBS/TEAMS
You will need to complete a club and a team application form for each team applying to compete, which can be obtained by downloading a copy from the website (see 'Application Forms' on the menu) or by emailing elaine.pritchard2@ntlworld.com or ringing Elaine on 07718 688662 or Malc on 07751 488523.
Both the club and team applications forms should be sent to the League Secretary; Elaine Pritchard, 3 Hathersage Way, Sutton in Ashfield, Nottinghamshire. NG17 2BE by the closing date which is 1st July 2009.
When your club application form is received, it will be checked for completion in full and acknowledged within 5 days. Please note that it will be returned at this stage if it is not signed by the Club Secretary because it will be invalid (no pp's please). A ‘Registration Pack’ will then be prepared, which will include 14 player registration forms and a leaflet from Notts FA explaining the registration system and what you need to do. This will be given to you at our Annual General Meeting to be held on Thursday 18th June 2009 at 7.30 pm at The Bentinck, 20 Sutton Road, Kirkby in Ashfield, Notts. NG17 8GS.
PLEASE NOTE; YOU WILL NOT BE ENTITLED TO VOTE AT THE ANNUAL GENERAL MEETING UNLESS THE LEAGUE IS IN RECEIPT OF YOUR CLUB APPLICATION FORM FOR THE SEASON 2009/10.
All Clubs are required to affiliate to their own County F.A. and for Nottinghamshire Clubs this means you will not be able to play in the league unless you have completed 'Form A'. You can obtain 'Form A' directly from Notts FA by ringing 0115 9837400 from Monday to Thursday 9.00 am to 3.30 pm or by emailing peter.burton@nottinghamshirefa.com or info@nottinghamshirefa.com or ring 07751 488523 for a copy. When completed, this should be sent directly to the Nottinghamshire Football Association with the appropriate fees including affiliation, personal accident and public liability insurance premiums, to be received by them no later than 1st July 2009.
PLEASE NOTE; YOU WILL NOT BE ABLE TO AFFILIATE A TEAM NEXT SEASON UNLESS YOU HAVE A CLUB WELFARE OFFICER WHO HAS BEEN CRB CHECKED THROUGH THE FA MEDIA GROUP, AND HAS ATTENDED A CHILD PROTECTION BEST PRACTICE WORKSHOP (CPBP) AND A WELFARE OFFICER WORKSHOP (WOW). For Clubs outside Nottinghamshire you will need to affiliate through your own County F.A.
In all cases you should notify the League Secretary of your affiliation number by 1st August 2009.
Notts FA require completion of 'Form D' by the League before the commencement of the season so that they can cross check all the teams who have applied into the KicKStart League with the affiliation forms they have received from Clubs, to make sure that all the teams in each age group have been covered by insurance on the affiliation form.

FOR EXISTING CLUBS/TEAMS
A letter has been sent out this week 13th May 2009 to Club Secretaries of all teams who have played in the league this season, containing all the information and documentation you will need to re-apply next season.
The documentation will include your:-
* Club and Team Application Forms which should be returned to the League Secretary by 1st July 2009. If you require more team forms you can download one from the website (see 'Application Forms' on the menu), photocopy the original or you can e-mail or ring me or Malc anytime;
* A leaflet from Notts FA for each of your Managers, explaining the registration system and what you need to do;
* A list of all your existing players who need a new photograph next season (to be returned with your re-registration form to me please);
* Re-registration forms for all the teams you had registered last season. These should be signed by both the players staying with you next season and their Parent/Guardian, then checked, signed and returned by you immediately. It’s as easy as that. If you complete and return this form ON OR BEFORE 18TH JUNE 2009, YOU WILL NOT NEED TO PROVIDE ANYTHING ELSE unless of course some of your existing players need a new photograph as outlined above.
After this date, you will need to complete a new player registration form for each existing player and return this complete with two photographs and a copy of their birth certificate, as you would for a new player. This has been introduced to encourage you to send your re-registration form back immediately to ensure registration cards can be produced on time.
PLEASE DO NOT HOLD ON TO YOUR RE-REGISTRATION FORM WAITING TO GET IT COMPLETED BY ALL YOUR PLAYERS. IF YOU CANNOT GET IN TOUCH WITH ONE OR TWO OF THEM FOR SOME REASON, SEND THE FORM BACK AND COMPLETE A NEW PLAYER REGISTRATION FORM FOR EACH MISSING PLAYER. IF THIS FORM IS RETURNED ON OR BEFORE 18th JUNE 2009, AGAIN YOU WILL NOT NEED PHOTOGRAPHS OR BIRTH CERTIFICATES. AFTER THIS DATE, YOU WILL.
* New player registration forms will be available after 1st June 2009 on request to enable you to sign new players (please note new player registration forms will not be available before this date because they are not free agents until then);
ALL FORMS, BOTH RE-REGISTRATION AND NEW PLAYER REGISTRATION NEED TO BE SENT BACK TO THE LEAGUE SECRETARY AT 3 HATHERSAGE WAY, SUTTON IN ASHFIELD, NOTTINGHAMSHIRE. NG17 2BE
All Clubs are required to affiliate to their own County F.A. every year and for Nottinghamshire Clubs, this means you will not be able to play in this league or any other league unless you have completed Form A, which I have sent for your convenience.

PLEASE NOTE THAT IT IS A REQUIREMENT THAT CONTINUING MEMBER CLUBS ATTEND THE ANNUAL GENERAL MEETING, IN ACCORDANCE WITH LEAGUE RULES.
YOUR CLUB WILL NOT BE ENTITLED TO VOTE ON SOME MATTERS AT THE ANNUAL GENERAL MEETING UNLESS THE LEAGUE IS IN RECEIPT OF YOUR CLUB APPLICATION FORM FOR THE SEASON 2009/10.
On receipt of your Club Application Form, if you have applied to enter additional teams next season, you will be provided with a 'Registration Pack' for each team, at the Annual General Meeting.
Your focus initially should be on getting your Club and Team Application Form(s)and also your re–registration form completed and returned to me as soon as possible.
