As per Orange County Youth Football League (OCYFL) rules every player or cheerleader must provide medical consent from their doctor dated in the 2009 calendar year to participate.  This is an insurance regulation that is handed down to us.  If you have a physical dated from 2008, regardless of the month it was completed,  it is not valid for the 2009 season.  You do not need to submit an actual physical from your doctor for your child to participate.  In the past this has posed a problem because families cannot get annual physical appointments until after football starts. 

 

To help alleviate this problem, the OCYFL has provided a Medical Consent Form that you can download and print.  You can bring this form to your doctor at any time before registration and have it filled out if you are unable to obtain an actual physical for your child.
