CODE OF CONDUCT AND RELEASE OF ALL CLAIMS AGREEMENT
WEST MILFORD WOLVERINES 11U BASEBALL CLUB --- CODE OF CONDUCT AGREEMENT AND RELEASE OF ALL CLAIMS RELATING TO PARTICIPATION WITH THE WEST MILFORD WOLVERINES 11U BASEBALL CLUB:
It is understood and acknowledged that by signing the within release form, we release any and all claims against the WEST MILFORD WOLVERINES 11U BASEBALL CLUB, its individual manager/coaches and administration.
It is mutually agreed that in consideration for the signing of the within release of all claims and code of conduct agreement, the undersigned will be afforded the privilege and opportunity to play baseball with the WEST MILFORD WOLVERINES 11U BASEBALL CLUB. Said privilege requires the individual player, parents and siblings, relatives and spectators to acknowledge the principals of the league and the concepts itemized herein.
The CODE OF CONDUCT AGREEMENT of the players, coaches, administration, parents, all family members and spectators involved to emphasize those values, principles, beliefs which are central to the foundation, structure and operation of the WEST MILFORD WOLVERINES 11U BASEBALL CLUB are itemized herein as follows:
At all times, we agree to and support:
It is the purpose and intent of the WEST MILFORD WOLVERINES 11U BASEBALL CLUB to create and offer quality learning and baseball playing opportunities.
Positive and exemplary relationship, and appropriate demeanor by everyone, including parents and relatives.
Safety and well being of players, coaches, umpires and spectators.
First class representation of players, coaches, umpires and spectators.
Full respect for the presentation of our flag, pledge of allegiance and national anthem.
Always playing within the established rules of the game (i.e.: age limits, etc.) and particular regulations established by individual coaches.
To acknowledge, respect and honor all local town rules and regulations as it pertains to the field of play.
The understanding that participation with the WEST MILFORD WOLVERINES 11U BASEBALL CLUB is a privilege and a player/family decision, completely voluntary, without pressure or coercion ... as such, dual/multiple participation questions or consequences (with other baseball teams, leagues or organizations) are 100% family matters and family responsibilities.
Physical, mental and spiritual preparation and fitness.
Realization that all of our activities are designed to enhance growth and development of all our players, families and coaches.
Mutual respect, tolerance, understanding and caring in all team matters.
Coaches' decisions (... whether we understand, like or agree with them!) are being made within the parameters of established rules regulating same.
Emphasis on team spirit, team unity, team effort and team success.
Encouragement of everyone involved ... players, coaches, family members, etc.
The reality that some players may have more plate appearances, playing time, position/roles and starting assignments than others.
Tactful and private (one-on-one) discussions of any concerns with team manager/coaches.
Acknowledgement that each of us is responsible for our own thoughts, feelings, attitudes, decisions and behaviors.
Giving 100% of our attention, commitment and effort to our team, and therefore, to our team goals.
Importance of leaving each other, each dugout, each ball park, each tournament site and each situation in better shape than we found them.
Outstanding player talent, effective/experienced coaching and top competition.
The clear understanding that all medical, financial and legal liability issues, activities and consequences are the exclusive responsibility and domain of the player and/or his respective family, parent(s), and/or guardian ... at no time will the WEST MILFORD WOLVERINES 11U BASEBALL CLUB nor any of its associated personnel (founder, manager, coaches, board of directors, etc.) be sued legally (individually, collectively or organizationally) in any matter, for any expenses, damages (perceived real, intentional or unintentional), losses, injuries, complaints/problems, complications and/or inconveniences which result from participation with the team activities and/or team initiatives. We agree to release and forever hold harmless said team and related personnel in all legal matters, legal, medical, financial and/or otherwise.
No claims shall be made for any reason.
We have read, have discussed and understand this RELEASE AND CODE OF CONDUCT AGREEMENT, and fully, unequivocally support it, its intent and its essence.
