Uniform Orders:
If this is your first or second year with the East Islip Ice Hockey Club, you are entitled to one practice Jersey as part of your tuition. Please check with your team coach to see if there is a preference on color. You should order the practice jersey with your uniform order, but don't have to include the amount of the parctice jersey in the payment. You will not get a practice jersey as part of the order unless it is put on the order form and returned by the deadline. (Even if this is the only thing you are ordering)
For all other club members, those who are entering their 3rd season or more.
If you need a new practice jersey, they can be ordered with your uniform/apparel order. The cost of the practice jersey is $16.00. Please include the payment with your apparel order.
Please check with your coach to see if there is a preference on color.
Below is the uniform ordering form which should be printed and filled out and brought to practice. We will have jerseys for fitting those who do not have game jerseys and covers as well.
