Falcons 2009 Registration Form
We are currently accepting registrations for the 2009 season.
A complete registration consists of the following:
Completed Registration Form
Copy of the Child's Birth Certificate
Medical Release Form
-A physical is good for 2 years, but the form must be signed off by
physician each year.
-A signed copy of the child's physical is also acceptable.
Copy of the Final 2008-2009 Report Card
Once we have received all of the above information, we will load your child's registration in to the NHYFSC web site. We will print off a copy, which will require your signature, for the team books. We will also take a photo of your child to put with their registration in the book. The NHYFSC will then review all of the information and approve it.
New for 2009 is Flag Football for 5-7 year olds. We are currently looking for coaches for this level.
