The Below Information Is For Our Members Who Have Volunteered To Coach

PSP Criminal History & Childline Instructions & forms
MANDATORY
FOR ALL COACH's & ASSISTANTs
Per State, Township, & Kedron Youth Association requirements,
ALL sport coaches & anyone designated as an 'assistant' are required to have a criminal background check thru the Pennsylvania State Police (PSP) and Childline.
This does not include parents who help out on a day to day basis at practices and/or games.
Due to privacy laws,
this entire process MUST be completed by the individual Coaches & Assistants,
and cannot be done by the Kedron Youth Association.
This is a two step process which must be done as soon as possible and will cost each coach and/or assistant a total of $20.00.
* THE FIRST STEP *

MUST BE COMPLETED PRIOR TO THE PLAYING OF ANY GAMES
This can be done immediately by clicking above

Choose " VOLUNTEER " as the Reason For Request
Complete the rest of the required fields
A credit card will be required and a $10.00 fee will charged
PLEASE KEEP THE RECORED OF YOUR RESPONSE FOR DOCUMENTATION PURPOSES
If you are unable to complete the above process online,
or do not have a credit card,
you may download, print, complete and mail the
SP 4-164 Form & PA Child Abuse History Clearance <~~~~~By Clicking here
Payments MUST be made by separate Money Orders when mailing.
Personal Checks Are Not Accepted.
If you opt to mail the form into Harrisburg,
it will take longer to receive your record check and clearance.Clicking the
SP 4-164 Form & PA Child Abuse History Clearance link and downloading the instructions and forms will allow you to complete BOTH the PSP Criminal Record Check as well as the PA Child Abuse History Clearance check through Childline.
These forms will require separate Money Orders, and to be sent to different Agency addresses.

PLEASE READ THE INSTRUCTIONS AT THE TOP OF THE FORM
This Form will require a separate Money Order payable to the Department Of Public Welfare
And to be sent to the address in the top portion of the form
Due to the high volume of requests at certain times of the year,
we suggest taking your child abuse history clearance applications to your state representative office for delivery to Harrisburg.
Their office can expedite requests for child abuse history clearance for an individual.
They CANNOT forward PSP Criminal Record Checks
(Which can be done online)
KYA President Dave White has offered to coordinate with our State Representative's Office the delivery of the child abuse history clearance applications.
You MUST properly complete the form, enclose your money order payable to the Department of Public Welfare, and enclose them in a sealed envelope.
