STEP 4: 2009-2010 ONLINE REGISTRATION FORMPlease click on the above link and submit the completed registration form. A confirmation of your registration will automatically be
e-mailed to you, Tom Parkins (League Rep.) and Don Brenner (President)
A separate form must be filled out for each player wishing to participate
STEP 5: TRYOUT & COMMITMENT FEES (checks payable to Plum Hockey Association)
