USA Hockey Accident Insurance Claims Procedure
January 15, 2003 --
Membership in USA Hockey includes coverage for injuries suffered while playing/practicing/coaching with your team in sanctioned events. In the event of an actual injury, or an incident involving a possible injury, the following procedures should be used to report these injuries to the proper claims personnel. All claims, however minor they may appear, should be reported so that we can assist the injured person and use the information to prevent other such incidents.
PLAYER & COACH EXCESS CLAIMS
All claim reports should be made to the injured person's primary personal or group health insurance company first, for determination of benefits. Only after the injured person has reported the injury to their primary provider, may they contact USA Hockey's claims unit (for participant accident/excess medical assistance) at the number shown below:
