SF Rebels Program - Transitional Services

The first component of the San Francisco Rebels Program are transitional services.

Transitional Services

•Linking youth with services that will deal with their quality of life issues
•Referal and case management
•Care plan will be devised by program case manager to address placing youth with service providers, i.e. mental health, substance abuse
•Each participant will receive an assessment and extensive supervision
•Compile monthly data on individual
•Serve as a liaison between youth, family, schools, probation officers and courts
•Conduct workshops that address such issues as conflict resolution, gang prevention, money management, and employment.
•Grief counseling
•We help youth that are phasing out of the foster care system adjust to their new life by providing case management services including vocational, educational, employment, housing services, relapse prevention, and family therapy.
•Each youth will stay in program 3-6 months. A follow up will be done after completion of program and assessment will be done to determine if additional services are needed.
•The goal for each case manager is to build a relationship of trust and mentor the youth while meeting tangible needs.

SF Rebels Program - Home Based Supervision

The second component of the San Francisco Rebels Program is home based supervion.

Home Based Supervision

•Take referrals from community, courts, schools, individual and family referrals.
•Curfew checks (random, physical, phone checks and email)
•Serve as a liaison between youth, family, schools, probation officers and courts if needed
•Provide job placement
•Provide case management services including vocational, educational, employment, housing services, relapse prevention, and family therapy.

SF Rebels Program - Academics

The third component of the San Francisco Rebels Program is academics.

Academics

•Address truancy by tracking/monitoring attendance patterns•
•Parent education
•Identify obstacles that are impeding youths ability to go to school
•Assist with academic advising
•Provide educational advocacy and direction leading students toward graduation
•High School prep workshops
•Provide resources and referrals to other programs or community based organizations that will assist youth in their academic success.
•Re-engage, re-enroll, re-motivate youth towards an educational goal.
•Tutoring and mentoring
•Serve as a liaison between students, teacher’s education and community-based organizations.
•Work in collaboration with parents/caregivers on their child(s) progress, challenges and academic success.

SF Rebels Progam - Athletics

The fourth component of the San Francisco Rebels Progam is athletics.

Athletics

•Address the growing obesity epidemic facing our youth by focusing on healthy eating habits and active lifestyles
•Foster team work through athletics
•Model good work ethic and teach youth to positively interact with other youth through athletics
•Build personal confidence through play
•Teach youth dynamics of winning and losing and to keep both in perspective
•Provide affordable organized athletic program to economically disadvantaged youth
•Incorporate values of education; motivate positive youth leadership and empowerment.
•Provide a supportive network of adults to have a positive influence on youth
•Build a sense of pride in self and community in each of our youth
•Teach our youth to be responsible for their actions
•The program provides youth the option to participate in physical activities beyond the normal school day, promoting healthy lifestyles and a means to learn teamwork, sportsmanship, and fair play.
•Facilitate mentorship programs for our younger members from our teen members to establish commitment and service.
