WEST COVINA UNIFIED SCHOOL DISTRICT
Use of WCUSD property is a privilege not a right. Commitment to civic pride, in recognition of the benefits whenever we use school property, and as a measure of gratitude to the school for allowing us the use of the facilities during any league event facilities, we agree to enforce this all school district provisions.
As a member of WCYBL, entrance into any open building on school district is prohibited. Urination on school district property is also prohibited. Make sure your players are using the bathroom prior to practices.
Each team is required to donate clean-up time on the courts and in and outside of the gym whenever they are on any school property. At the conclusion of all practices and games, each player is mandated to pick up trash and litter.
The home team of the last game of the day will be responsible for cleaning the bleachers and assisting Safety and Facilities in any other duties. The visiting team of the last game of the day will be responsible for picking up loose trash on the floor and around the perimeter of the gym.
Any team found guilty of not meeting this requirement faces disciplinary actions by the board which may include suspension of practice courts or further steps to and including probation of the coach.
CITY OF WEST COVINA, COMMUNITY SERVICES DIVISION, CAMERON PARK COMMUNITY CENTER
In conjunction with West Covina Youth Basketball League (WCYBL), I do hereby allow my child to participate in games at the Cameron Park Community Center while participating as a member of West Covina Youth Basketball League. I fully understand that WCYBL bylaws, rules, policies, and procedures (written and spoken by the Board of Directors) must be adhered to at all times as part of my league membership and as part of my child’s participation. WCYBL Board of Directors and special volunteers will only be responsible for the general supervision and maintenance of the gymnasiums, snack bar, and restrooms. Any child/parent/legal guardian that chooses to participation in recreation activities at the community center or at Cameron Park, releases WCYBL Board of Directors, special volunteers appointed by the president, head coaches, assistant coaches, and team managers of any liability, responsibility, or accountability.
In addition, I hereby release, discharge, and agree to “indemnify” the City of West Covina, its officers, agents, and employees from against all actions, claims, or demands that I, my heirs, distributes, guardians, legal representatives, or assigns have, or now have from today, for injury, death, or damage resulting from their participation in recreation activities at Cameron Park Community Center.
I have carefully read this agreement and fully understand its contents. I am aware this is a release from liability and a contract to indemnify the City of West Covina, WCYBL, and myself, and I sign it of my own free will.
Cameron Park Community Center Rules
• Board of Directors and members of the coaching staff must have their league identification card at all times.
• WCYBL bylaws, rules, policies, and procedures will be enforced at all times.
• WCYBL participants are not allowed in the multipurpose rooms unless accompanied by their Coach and/or a member of the Board of Directors and only with permission of the community center staff.
• NO FOOD, DRINKS (EXCEPT WATER), GUM, CANDY, SUNFLOWER SEEDS, OR SPORTS BOTTLES WILL BE ALLOWED IN THE COMMUNITY CENTER GYMNASIUMS.
• Conducting league practices at the community center is prohibited without prior approval of the Board.
• Noise levels in corridors and foyers should be kept to a minimum.
• Entering the community center staff offices is prohibited; sitting or loitering on counter-tops is prohibited.
• Participants must use front doors, except during emergency evacuation.
• Upon entering the facility, you grant permission for the League and/or City to use photographs of you and/or your child taken during activities or events for League and/or City purposes.
• This facility is equipped with security cameras to monitor activity.
• Bad language, fighting, horseplay, and gambling are not permitted.
• Please be respectful of the facility, participants and staff.
• No smoking within 20 feet of the facility.
• Only staff may change the television channels.
• City is not responsible for participant belongings; lockers are available for participants at their own risk.
• Please pick up after yourself.
• WCYBL is not responsible for the game area or children playing in the game area.
• Obey West Covina City staff and volunteers directions.
This form was approved by the West Covina Community Services Department. Failure to abide by any of the times listed above and any other WCYBL rules will result in suspension from the Community Center and ultimately dismissed from WCYBL.
