Record Release Form
Student use this form to get a release of school records from previous school
Emergency Information Form
In the event emergency medical care must be provided for your student, a copy of this form will be given to the care provider, i.e., paramedic, hospital, doctor, dentist.
Recommendation Form
This form needs to be taken to your previous school and filled out by an administrator.
Association Churches Discount
This form should be downloaded by Members of our Association Churches and given to your pastor to complete and return to Lutheran High School.
Transcript Request Form
This form is necessary to order transcripts for college or other related business.
