Registration Requirements - All players must submit the following forms by the 1st day of practice:

1) MYLC Code of Conduct Form

2) MYLC Waiver & Release Form

3) Proof of US Lacrosse Membership

 

2020 Code of Conduct Form  

Players must submit completed form to Head Coach prior to 1st practice.  Players will NOT be allowed to participate until all paperwork is submitted.

Proof of US Lacrosse Membership

All MYLC Coaches/Players/Board Members must have a US Lacrosse membership that is valid through June 30th, 2020.   US Lacrosse Membership provides General Liability and Accident Medical Insurance to all members while participating in covered amateur lacrosse activities. 

Proof of US Lacrosse membership must be submitted to the Head Coach prior to participation.  Players/Coaches/Board Members will NOT be allowed to participate until proof of US Lacrosse Membership is provided.

 

 

MANDATORY Baseline Concussion Screening 

 

We are proud to continue MYLC's mandatory concussion baseline evaluations for the boys 10-years-old & up ONLY AND optional testing for the girls 10-years-old & up ONLY.  Baseline concussion evaluations are conducted when athletes are healthy to capture their normal function.  Baseline concussion evaluations are a tool to help medical professionals determine if/when athletes who have experienced a concussion are healed and ready to return to sports.  

 

MYLC works with Sports Concussion Center of New Jersey to provide a baseline evaluation for $40 (normally $78).  Several dates will be available in February to take the hour long evaluation. Paperwork will be disseminated prior to the appointment for a smooth process at their convenient Princeton office.

If your family prefers to use a different testing facility, please provide proof of baseline concussion screening to info@montgomerylacrosse.com.  

 

Baseline concussion screening must be conducted prior to player participation.

 

If you believe your child may have a concussion, please seek immediate medical attention from your family physician or an emergency medical facility.

 

NOTE:  If your child was screened for the Pop Warner 2019 Fall season, please notify info@montgomerylacrosse.com. 

Injury Return Form.

This form must be completed and returned to the Head Coach prior to an injured player resuming participation.  Players will NOT be allowed to resume play from injury unless this form is completed and submitted to Head Coach.
