MY CHILD HAS BEEN INJURED PLAYING SOCCER AT PRACTICE (OR A GAME), WHAT DO I DO?
First, get the needed medical attention.
All MSA players, coaches, assistant coaches and other rostered individuals are covered by a private insurance that will supplement your personal medical coverage. There is a $500 deductible, and the coverage is 80/20.
Second, print out the 3 forms you will need:
1. Claim Form (needs to be signed by coach and parent and explains the coverage)
2. Injury Verification Form (needs to be signed by Vice President or Registrar)
3. Checklist Form
After signing, send everything to North Texas. Remember to keep a copy!
