2013 Registration Procedures
To play on the team in for the spring you must complete the required registrations. You must register with:
1. US Lacrosse - You must maintain a current US Lacrosse membership
2. Arizona Lacrosse League - Registration must be completed by 12/31/12
3. Aztec Lacrosse Club - All of the forms are below

The 2011 Tax Credit forms must be turned in to the bookstore before the school goes out on winter break. Make sure to write down CdS Boy's Lacrosse for the group.
The Aztec Lacrosse Club does not issue any refunds. The team dues are due in installments as the club makes payments to support the team. Based on this timeline we cannot issue any refunds for team dues that are submitted with registration.
