• Junior must present a signed medical release form. 
• The parent may sign a medical release on the day of the tournment.  No one other than the parent may sign the medical release.  If parent is not present the medical release must be notarized.

***NOTE: There must be at least 6 Teams Registered in the Mens' Open and Womens' Open to Qualify for $500 Cash Prize. If there are less than 6, 100% of total fees from open will be paid out to winners.*****
