Requirements for Eligibilty/Outline of Scholarship

• Applicant must meet AYF requirements to participate
• Applicant must carry a minimum 3.0 GPA as evidence on the report from the most recent trimester of school.
• Provide a brief autobiography of applicant (approx. 50 words)
• Provide any letter(s) of recommendation
• Optionally provide a letter from the applicant
• Provide proof of Financial need (see examples below)
• Minimum Volunteer requirements: 4 hours Partial/8 hours Full
• Scholarship covers registration fees only
• Mandatory Fundraiser is not covered by this scholarship
• Equipment deposit is not covered by this scholarship

Examples of Proof of Financial Need:

1. Approval letter from School District for FREE or REDUCED Lunch for the child

2. Section 8 status letter

3. State or County funded child care program

4. WIC

5. Foster child assistance

6. Medicare

7. Welfare or Food Stamps

8. or any other program that’s legitimately funded by the county or state.

All required information must be submitted prior to the registration deadline (last registration for payment of all funds).
