REGISTRATION IS OPEN FOR ALL PLAYERS!  To get to our online registration click on the 2017 ONLINE REGISTRATION tab to the left. 

Required Fundraiser Buyout: You buy 20 - $5.00 raffle tickets.  When you receive the tickets if you choose to sell them you keep the money. 

Registration Fees:

Smurf $150.00

Rink, Pony, Midget  $170.00

Multiple player discount (2nd player) $105.00, (3rd +) $85.00

Mandatory personalized jersey $35.00

Oldest player will be considered 1st player

Additional fee of $35 will be added if registered after April 30th

All families -  $100 for raffle ticket buyout (1 per family)

DIVISIONS - Age by August 31, 2018

Smurf - 6 and first year 7 year olds

Rink - 7 & 8 year olds

Pony - 9 & 10 year olds

Midget - 11 & 12 year olds

Player's Birth Certificate (Actual certificate - we can not accept Notice of Live Birth) - New players only

Copy of Health Insurance Card - New players and only if changed for returning players

Copy of Players Report Card

2 Copies of Proof of Residence

These items can be scanned and emailed to Shelby Ehrlinger @saehrlinger@gmail.com  

The completed Physical Form must be returned prior to first practice (form is located under Registration Forms tab)

Families are required to print 10 sponsor letters (located on the Registration Forms tab)  and complete the letters and address 10 envelopes with addresses and postage, where we will send the letters.  DCF will take care of sealing and mailing the letters out. Please do not mail these out on your own, only place postage on the addressed envelopes. These need to be completed and handed in for players to receive equipment.

GENERAL INFORMATION

Practices begin August 8th and will be Monday-Friday from 6PM-8PM until school starts then practice will be Tuesday-Thursday.

Practice times for Tuesday-Thursday practices: Smurf 6PM-7:30PM, Rink 6PM-8PM, Pony 6PM-8PM, Midget TBD

We supply all equipment except for cleats and athletic supporter (cup).
