Please list uniform and sock sizes on the reverse side of this form. Refer to uniform sizing guidelines for proper measurements. Please print clearly as this information will be entered into the registration database and will be the primary source of contact information. Email address is vital to consistent communication throughout the fall season. Registration fee and copy of birth certificate for first year players is required to complete the signup process.
2008 Age Requirements
All players must be 8 years old by July 31st, 2008 and not older than 13
by July 31st, 2008.

Assesment/Tryout For Multiple Teams in One Age Group
Team Placement Guidlines(U12 thru U14) for Multiple Teams in One Age Group
U9 thru U11-Rosters for multiple teams in one age group will be up to the discretion South Portland Strikers Board of Directors.
U12 thru U14-Assessment/tryout will be held to determine rosters for multiple teams in one age group. Tuesday, June 3rd and Tuesday, June 10th. 6:15pm-7:15pm. Wainwright Recreation Complex. Quad immediately after baseball field along the Greenbelt Walkway. Please check website June 1st, 7:00am to verify if registration numbers dictate an assessment/tryout is necessary to select multiple rosters. Email will be sent out to participants. One group will be assigned to the Challenge Cup division and the second to the Anthem Cup division.
***The South Portland Strikers Board of Directors reserves the right to finalize any rosters in the best interest of the organization as a whole separate from any item listed in the Team Placement guidelines at any point in time.

Coach, Asst. Coach & Team Manager Background Checks
All Coaches, Assistant Coaches, & Team Manager's will need to complete the online Soccer Maine Risk Management(volunteer registration)by May 31st. A complete background check will be completed on all individuals that would like to have a team coaching/manager position. Anyone not completing this process and is not part of the team roster can not be on the sidelines of any game or practice. Only 3 adults can be on the sidelines during any one game. Volunteer positions will be assigned pending a successful Risk Management application.
