Volunteer Form
This document (Bollingers Guide) includes these forms: Employee/Volunteer Affidavit, Employee/Volunteer Application, and the Zero Tolerance Policy Against Abuse. The Volunteer Application is located on pages 19 & 20. Anyone interacting with the team must sign this form (scorekeeper, team parent, etc.)
All team managers, coaches, assistants and helpers (anyone on the field) must be ACE trained. Please go to the LINKS page for the ACE training link.
